David, a month after my husband, his mother, and I completed our Advanced Directives, his mother had a stroke. It was a tremendous comfort to us to know that we had the ability to make good decisions on her behalf. The stroke left her unable to speak or swallow, and she refused the tube they installed through her nose—kept pulling it out. We approved their putting in a “peg” through her abdominal wall into her stomach—because, while her AD indicated “no artificial feeding,” we knew the context in which she meant that. We also knew that we had the power to make decisions for her that we believed to be in her best interest. She spent six weeks in rehab, getting intense therapy for swallowing, physical therapy, and occupational therapy, and then we brought her home. She still has the peg, but it’s a supplementary item, mainly to ensure hydration. She is back to eating food, getting better at it every day. The peg is also a very minor item to manage, really a non-problem.

The best thing I did was show the neurology team in the ER videos of my mother-in-law cutting up at a family party, three days before her stroke. “Let me introduce you to my mother-in-law,” I told everyone who cared for her. She was waving a funny hand clapper with the great-grands, using her right arm—the arm that was incapacitated from the stroke. “This is clinically significant, isn’t it?” Those photos and videos from the party gave them a window into her real life and the impact the stroke was having. I believe that it galvanized the team, though I can’t prove it.
